Physical examinations will be offered for middle school and high school athletes on Aug. 3 from 8 a.m. until noon. Examinations will be on a first-come, first-served basis and will take place at Southwest Memorial Primary Care, located at 33 North Elm Street in Cortez.
Athletes must have a signed "consent to treat a minor" form and a health history form in order to be treated. Forms are available online at www.swhealth.org or at any area primary-care clinic.
The cost of a physical examination is $30. Checks should be made payable to Southwest Health System, Inc. Insurance is not accepted for physicals, and payment is due at the time of the visit.
