The Mancos School District is pleased to announce that we will be offering Sport Physicals/Summer Camp Physicals in cooperation with the medical providers from Southwest Health Systems/Mancos Clinic. This clinic will be held on campus during regular school hours on April 18. These physicals will be open to all grades K- 12. The cost will be $20, of which $10 will be generously donated back to our school's athletic department.
Paperwork and forms are available at school and MUST be signed by a parent/guardian, and returned to the school nurse no later than April 17, along with a prepayment of $20 in cash or check.
A sport physical form must be on file prior to participation in athletic events including practice which generally begins mid-August.
